Upon completion of the application, our conference administrator will send instructions with codes required to complete the two online assessments (StrengthsFinder & DiSC). Completion of these assessments are due within one week of the application submission.
Pay online by credit card (option available after completing application) or by check on the day-of LA.
Note: Once you apply you are responsible for covering the cost of the assessments ($35). If you decide not to attend LA on or before 12/18/09 you can be reimbursed everything except for the assessment cost. After 12/18 your registration cost is non-refundable as we are then committed to cover all other costs associated with your registration. Calvary does not make a profit from Leadership Advance. We are committed to keeping the costs as low as possible without compromising quality.
Participants: The application will take approximately 45 min. or more to complete. Please allocate enough time as you cannot save your responses and re-enter.

Coaches: Please fill in all required responses (even past coaches as your
contact information might have changed). The other questions are
optional but are available so you are familiar with what participants
are asked. Coaches are asked to only cover the cost of assessments. DiSC ($20) and Strengths Finder ($15). If you have taken these assessments previously please provide us with your results.
